Case Notes are written records therapists keep about client sessions, like a summary of what happened, goals, and progress. Examples include SOAP notes (Subjective, Objective, Assessment, Plan) and DAP notes (Data, Assessment, Plan) and more.<\/p>\n\n\n\n<p><\/p>\n\n\n\n<p>If you are a SafeTalk User, we have <em>digitized &amp; automated <\/em>the process of writing and storing <em>Case Notes<\/em> for you. Manage your notes at anytime from anywhere with any device. Let&#8217;s dive in!<\/p>\n\n\n\n<div style=\"height:1px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<div style=\"height:40px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p><\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"team-managers\">For Team Managers: Templates for your Team<\/h2>\n\n\n\n<p>As an Organization Manager\/Owner, you have the ability to create Templates that can be used by anyone within your team. <em>Let&#8217;s do that!<\/em><br><br><strong>Step 1: <\/strong>From the left sidebar, go to <strong>&#8220;Organization&#8221;<\/strong> page &amp; select  the &#8220;<strong>Templates&#8221; <\/strong>tab from the top menu<\/p>\n\n\n\n<p><strong>Step 2<\/strong>: On this <strong>&#8220;Templates&#8221; <\/strong>page, it&#8217;s where you can manage(add\/edit) your <em>organization templates<\/em>. On this page you are able to manage(edit\/delete) your personal templates. These templates will be only visible and usable to <span style=\"text-decoration: underline\"><em>you<\/em><\/span>. Simply click on <strong><em>&#8220;Create New Template&#8221;<\/em><\/strong> button on top right to start adding your personal <em>templates<\/em><br><br><strong>Step 3.<\/strong> Go to &#8220;<a href=\"#setup-new-template\" data-type=\"internal\" data-id=\"#setup-new-template\">How to setup a new template<\/a>&#8221; section to learn how to setup new templates<\/p>\n\n\n\n<p><\/p>\n\n\n\n<figure class=\"wp-block-video rounded-2xl-custom\"><video height=\"1080\" style=\"aspect-ratio: 1920 \/ 1080;\" width=\"1920\" autoplay controls loop muted src=\"https:\/\/safetalk.space\/blog\/wp-content\/uploads\/2024\/02\/case-note-template.mp4\" playsinline><\/video><\/figure>\n\n\n\n<div style=\"height:24px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"setup-new-template\">How to setup a new Template<\/h2>\n\n\n\n<div style=\"height:0px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p><strong>Step 1: <\/strong> Click on <strong>&#8220;Create New Template&#8221;<\/strong> and select one of the options<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Create a template from Blank<\/strong>. <em>You are only given a blank page &amp; a number of tools such as font typography, tables, images and more. Design your templates from scratch to look exactly how you want to them to be. <\/em><\/li>\n<\/ul>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Duplicate Your Template<\/strong>: Want to duplicate an existing template you have? It&#8217;s easy, select the template you have in mind, click <strong>&#8220;duplicate&#8221;<\/strong> and you are good to go. <em>No changes will be made to the original template<\/em><\/li>\n\n\n\n<li><strong>Library<\/strong>: SafeTalk has you covered! We have created a library of various templates ready for your use. Duplicate our templates to your own library &amp; modify them to match your needs<\/li>\n<\/ul>\n\n\n\n<p>Once you have selected the <em>base <\/em>for your template, you will redirected to the template design page. Here you are given a number of tools(similar to Microsoft Word) where you can start designing your personalized templates. <em>Be Creative and Make your templates look custom to your practice!<\/em><\/p>\n\n\n\n<figure class=\"wp-block-video rounded-2xl-custom\"><video height=\"1080\" style=\"aspect-ratio: 1920 \/ 1080;\" width=\"1920\" autoplay controls loop muted src=\"https:\/\/safetalk.space\/blog\/wp-content\/uploads\/2024\/02\/case-note-template-new.mp4\" playsinline><\/video><\/figure>\n\n\n\n<p><em>Note: when creating your templates, on top left of the menu bar, you will see a dropdown list called &#8220;Custom Fields&#8221;. <a href=\"#custom-fields\">Click here to learn<\/a> about custom fields &amp; how they work<\/em>.<\/p>\n\n\n\n<div style=\"height:24px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"custom-fields\">Custom Fields<\/h2>\n\n\n\n<p>Our <em>custom fields<\/em> are created to make your note writing easier than ever. Use <em>custom fields<\/em> to auto-populate certain information inside your case notes. Example: if you add the &#8220;Client&#8217;s Fullname&#8221; custom field in your template, when using this template to create a <em>Case Note<\/em> for a <em>Client<\/em> the information will be automatically populated. You are given access to a number of custom fields, from Client Details (Full name, Contact No, Emergency Contact Details, Address, more) to Therapist Details (Full name, Professional License No, Signature, Organization Details, more) &amp; more.<\/p>\n\n\n\n<figure class=\"wp-block-video rounded-2xl-custom\"><video height=\"1080\" style=\"aspect-ratio: 1920 \/ 1080;\" width=\"1920\" autoplay controls loop muted src=\"https:\/\/safetalk.space\/blog\/wp-content\/uploads\/2024\/02\/case-note-custom-fields.mp4\" playsinline><\/video><\/figure>\n\n\n\n<p><br><em>Note: To add a custom field inside your template, click on &#8220;Custom Fields&#8221; dropdown, select a field you want to add &amp; simply click on it.<\/em><\/p>\n\n\n\n<div style=\"height:10px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"therapist-legal-info\">Therapist&#8217;s Legal Info<\/h2>\n\n\n\n<p>Optionally, you can pre-set your <em>Signature<\/em> &amp; <em>Professional License Number<\/em> as a <em>custom field<\/em> and seamlessly add them inside your <em>Client Case Notes<\/em>.<br><strong>Step 1:<\/strong> From the left sidebar, go to <strong>&#8220;My practice&#8221;<\/strong> page and select &#8220;<strong>Settings&#8221;<\/strong> from top menu<\/p>\n\n\n\n<p><strong>Step 2:<\/strong> Here you will see two fields at the bottom of the page called <strong><em>&#8220;Your Signature&#8221;<\/em><\/strong> &amp; <strong>&#8220;Professional  License Number&#8221;<\/strong>. Click on the fields and add your legal information here. <\/p>\n\n\n\n<p><em>Note: For Signatures, you can either create a digital signature or upload your signature as a file. Important: when uploading a signature .png format is recommended for the best result.<\/em><\/p>\n\n\n\n<figure class=\"wp-block-video rounded-2xl-custom\"><video height=\"1080\" style=\"aspect-ratio: 1920 \/ 1080;\" width=\"1920\" autoplay controls loop muted src=\"https:\/\/safetalk.space\/blog\/wp-content\/uploads\/2024\/02\/professional-settings.mp4\" playsinline><\/video><\/figure>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"writing-case-notes\">Writing Case Notes<\/h2>\n\n\n\n<p>Now that you have setup your templates, let&#8217;s get to what we are here actually for, learning how to write your <strong><em>Client Case Notes<\/em><\/strong> in SafeTalk.<br><\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"start-writing-case-notes\">Start writing your Case Notes<\/h2>\n\n\n\n<ol start=\"1\" class=\"wp-block-list\">\n<li>Open the <strong><em>Client History Page<\/em><\/strong> &amp; select the <strong><em>&#8220;Case Notes&#8221;<\/em><\/strong> tab from the top menu <\/li>\n<\/ol>\n\n\n\n<ol start=\"2\" class=\"wp-block-list\">\n<li>Click on <strong>&#8220;Add New Case Note&#8221;<\/strong> button on top right. You will be presented with the following options here:\n<ul class=\"wp-block-list\">\n<li><strong>Create a Note from Blank<\/strong>: where you can start a note on a blank A4 format page<\/li>\n\n\n\n<li><strong>Use a Template<\/strong>. Simple choose one of your templates and click <strong>&#8220;Use Template&#8221;<\/strong>.<\/li>\n\n\n\n<li><strong>Upload a File<\/strong>: You also have an option to upload a previous <em>Case Note<\/em> in any format to securely store all your Case Notes on SafeTalk.<\/li>\n<\/ul>\n<\/li>\n<\/ol>\n\n\n\n<figure class=\"wp-block-video rounded-2xl-custom\"><video height=\"1080\" style=\"aspect-ratio: 1920 \/ 1080;\" width=\"1920\" autoplay controls loop muted src=\"https:\/\/safetalk.space\/blog\/wp-content\/uploads\/2024\/02\/case-note-client.mp4\" playsinline><\/video><\/figure>\n\n\n\n<p>Upon selecting one of the options above, you will redirected to a <em><strong>&#8220;Write a Case Note&#8221;<\/strong><\/em> page. If you have selected a template with <em>Custom Fields<\/em>, all the information will be auto-populated. Add your notes and save your Case Note as a <em>draft<\/em> or <em>final<\/em>.<\/p>\n\n\n\n<div style=\"height:24px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"data-privacy\">Data Privacy &amp; Security<\/h2>\n\n\n\n<p>All Case Notes created in SafeTalk are protected with industry-standard encryption and stored on HIPAA- and PDPA-compliant infrastructure. Case Notes are <strong>not accessible to clients<\/strong>. They are only visible to you, the therapist. Access is shared <strong>only if you explicitly reassign the client to another therapist or choose to share the information<\/strong> as part of professional collaboration.
